"The one and only" is the third chapter of the 20th volume and the 173rd chapter overall of the Tokyo卍Revengers manga series, written and illustrated by Ken Wakui.
Summary[]
In a flashback, Shinichiro asks Mikey how he would feel about having another older brother. Mikey remembers this interaction as just Izana strikes him across the face; as Izana taunts Mikey, Draken remarks that Mikey is fighting at his maximum. Hearing this, Takemichi realizes Izana's strength, as Mikey is unable to attack Izana.

Kakucho explains Izana's strength
As Hanma and Kisaki watch the fight, Kakucho explains that Izana is used to fighting alone. When he was twelve, a group of boys ganged up on him, leaving Izana severely injured. After he recovered, he retaliated alone, attacking each person to the point of no recovery. Izana drove the group's leader to suicide by vowing to make his friends and family suffer while the leader was still alive. Kakucho says that Izana does not hesitate to kill, as he cannot stop himself from doing so; additionally, the power he holds in fights is a fatal force.

Izana says S62 fears him
Just then, Izana's punch throws Mikey to the ground again. Izana taunts Mikey again, disappointed in his power. As he tells Mikey again that he will kill him, Mikey stands, remarking that Izana does not care about anyone, not even his friends in the S-62 generation who fight for him. As Izana avoids another one of Mikey's kicks, he says that they are merely afraid of him and ideas such as friendship and trust are delusions. Takemichi notices that Izana is not landing his hits on Mikey and Draken agrees, explaining that Izana is getting tired, as his fighting is becoming sloppy.

Mikey tells Izana that they have each other
When Mikey asks Izana how long he will fight alone, Izana says Mikey is alone too, as his siblings are dead. While Mikey blocks Izana's kick, he tells Izana he is wrong, as they both still have a brother: each other. Thinking back to his conversation with Shinichiro, Mikey understands that they were talking about Izana. After Shinichiro asked him about having another brother, Mikey smiles and says he will definitely love him. Mikey tells Izana that he wants to save him.
